By age 50, the prevalence climbs past 40%. Most of them do not know it because metabolic syndrome rarely shows up as a single dramatic symptom. It quietly raises your risk of heart attack, stroke, and type 2 diabetes by 2\u20135\u00d7 while you continue to &#8220;feel fine.&#8221;<\/p>\n\n\n\n<p>India has the highest metabolic syndrome burden in the world, and the diagnosis criteria built for Caucasian bodies underestimate the Indian risk. It is a <strong>cluster of five risk factors<\/strong> that, when three or more occur together, dramatically raise your risk of cardiovascular disease, type 2 diabetes, and stroke.<\/p>\n\n\n\n<p>The five factors:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Abdominal obesity<\/strong> measured by waist circumference<\/li>\n\n\n\n<li><strong>Elevated blood pressure<\/strong>&nbsp;at or above 130\/85 mmHg, or on BP medication<\/li>\n\n\n\n<li><strong>Elevated fasting blood glucose<\/strong>&nbsp;at or above 100 mg\/dL, or diagnosed type 2 diabetes<\/li>\n\n\n\n<li><strong>Elevated triglycerides<\/strong>&nbsp;at or above 150 mg\/dL, or on lipid medication<\/li>\n\n\n\n<li><strong>Low HDL cholesterol<\/strong>&nbsp;below 40 mg\/dL in men, below 50 mg\/dL in women<\/li>\n<\/ol>\n\n\n\n<p>Three of these five = metabolic syndrome. The diagnostic criteria are from the International Diabetes Federation and the National Cholesterol Education Programme (ATP III).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why India has different (stricter) thresholds<\/h2>\n\n\n\n<p>The waist circumference thresholds were originally set for European populations. Indians, with our &#8220;thin-fat&#8221; phenotype, accumulate <a href=\"https:\/\/www.inbody.in\/blog\/visceral-fat-the-hidden-health-risk-how-to-reduce-it\/\">visceral fat<\/a> and develop insulin resistance at lower waist measurements than Caucasians. Now we sit for 10 hours a day and eat packaged food.<\/li>\n\n\n\n<li><strong>Carb-dominant diets<\/strong>, Indian thali typically runs 60\u201370% calories from carbohydrate, often refined (white rice, refined flour, sweets). This drives chronic insulin elevation.<\/li>\n\n\n\n<li><strong>Low protein intake<\/strong>: Typical Indian protein intake is well under the 1.0 g\/kg ICMR recommendation. Low protein \u2192 low muscle \u2192 low glucose disposal \u2192 insulin resistance.<\/li>\n\n\n\n<li><strong>Sedentary work + low NEAT<\/strong> desk work, commute by car\/scooter, lift instead of stairs.<\/li>\n\n\n\n<li><strong>Vitamin D deficiency<\/strong>&nbsp;over 70% of Indian adults are vitamin D deficient, which independently raises insulin resistance and inflammation.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Body composition: the missing piece of the metabolic syndrome puzzle<\/h2>\n\n\n\n<p>Standard metabolic syndrome diagnosis uses waist circumference as the obesity marker. The problem: waist circumference cannot distinguish between subcutaneous fat (less dangerous) and <strong>visceral fat<\/strong> (the metabolically toxic abdominal fat that wraps around your organs).<\/p>\n\n\n\n<p>This is why two people with the same 92 cm waist can have very different real risk profiles:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Person A<\/strong> 92 cm waist, mostly subcutaneous fat, visceral fat level of 8 on an InBody scan. Manageable risk.<\/li>\n\n\n\n<li><strong>Person B<\/strong> 92 cm waist, low subcutaneous fat but heavy visceral fat (level 14+ on InBody).
