By age 50, the prevalence climbs past 40%. Most of them do not know it \u2014 because metabolic syndrome rarely shows up as a single dramatic symptom. It quietly raises your risk of heart attack, stroke, and type 2 diabetes by 2\u20135\u00d7 while you continue to &#8220;feel fine.&#8221;<\/p>\n<p>India has the highest metabolic syndrome burden in the world, and the diagnosis criteria built for Caucasian bodies underestimate the Indian risk. This guide explains what metabolic syndrome actually is, who has it in India, how it is diagnosed differently for Indians, and what to do about it.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_is_metabolic_syndrome\"><\/span>What is metabolic syndrome?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Metabolic syndrome is not a disease in itself. It is a <strong>cluster of five risk factors<\/strong> that, when three or more occur together, dramatically raise your risk of cardiovascular disease, type 2 diabetes, and stroke.<\/p>\n<p>The five factors:<\/p>\n<ol>\n<li><strong>Abdominal obesity<\/strong> \u2014 measured by waist circumference<\/li>\n<li><strong>Elevated blood pressure<\/strong> \u2014 at or above 130\/85 mmHg, or on BP medication<\/li>\n<li><strong>Elevated fasting blood glucose<\/strong> \u2014 at or above 100 mg\/dL, or diagnosed type 2 diabetes<\/li>\n<li><strong>Elevated triglycerides<\/strong> \u2014 at or above 150 mg\/dL, or on lipid medication<\/li>\n<li><strong>Low HDL cholesterol<\/strong> \u2014 below 40 mg\/dL in men, below 50 mg\/dL in women<\/li>\n<\/ol>\n<p>Three of these five = metabolic syndrome. The diagnostic criteria are from the International Diabetes Federation and the National Cholesterol Education Programme (ATP III).<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Why_India_has_different_stricter_thresholds\"><\/span>Why India has different (stricter) thresholds<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The waist circumference thresholds were originally set for European populations. Indians, with our &#8220;thin-fat&#8221; phenotype, accumulate <a href=\"https:\/\/www.inbody.in\/blog\/visceral-fat-the-hidden-health-risk-how-to-reduce-it\/\">visceral fat<\/a> and develop insulin resistance at lower waist measurements than Caucasians. The IDF has issued separate Asian-specific cutoffs:<\/p>\n<table>\n<thead>\n<tr>\n<th>Population<\/th>\n<th>Waist (men)<\/th>\n<th>Waist (women)<\/th>\n<\/tr>\n<\/thead>\n<tbody>\n<tr>\n<td>Caucasian (US\/Europe)<\/td>\n<td>\u2265 102 cm<\/td>\n<td>\u2265 88 cm<\/td>\n<\/tr>\n<tr>\n<td>South Asian (India)<\/td>\n<td>\u2265 90 cm<\/td>\n<td>\u2265 80 cm<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>This means an Indian man with a 91 cm waist is already at metabolic syndrome risk territory \u2014 a measurement that would not have flagged him under the older Western criteria.<\/p>\n<p>Same story for BMI: WHO Asian-specific thresholds set overweight at BMI 23 (not 25) and obesity at 25 (not 30) for Indians.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Why_metabolic_syndrome_is_so_common_in_India\"><\/span>Why metabolic syndrome is so common in India<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<ul>\n<li><strong>Genetic predisposition<\/strong> \u2014 Indians have higher insulin resistance, more visceral fat, and lower lean body mass at the same BMI vs Caucasians.<\/li>\n<li><strong>Rapid lifestyle shift<\/strong> \u2014 three generations ago, most Indians did physical labour and ate seasonal home food. Now we sit 10 hours a day and eat packaged food.<\/li>\n<li><strong>Carb-dominant diets<\/strong> \u2014 Indian thali typically runs 60\u201370% calories from carbohydrate, often refined (white rice, refined flour, sweets). This drives chronic insulin elevation.<\/li>\n<li><strong>Low protein intake<\/strong> \u2014 typical Indian protein intake is well under the 1.0 g\/kg ICMR recommendation. Low protein \u2192 low muscle \u2192 low glucose disposal \u2192 insulin resistance.<\/li>\n<li><strong>Sedentary work + low NEAT<\/strong> \u2014 desk work, commute by car\/scooter, lift instead of stairs.<\/li>\n<li><strong>Vitamin D deficiency<\/strong> \u2014 over 70% of Indian adults are vitamin D deficient, which independently raises insulin resistance and inflammation.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Body_composition_the_missing_piece_of_the_metabolic_syndrome_puzzle\"><\/span>Body composition: the missing piece of the metabolic syndrome puzzle<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Standard metabolic syndrome diagnosis uses waist circumference as the obesity marker.
