The 2022 ESPEN–EASO consensus turned that definition into a usable pathway. You screen with a high BMI or waist circumference plus clinical suspicion. You confirm with a muscle-function test followed by body composition showing high fat with low muscle relative to weight. Then you stage the result. This guide sets out the sarcopenic obesity diagnosis criteria step by step, what the 2025 Asia-Oceania consensus adds, and where bioimpedance fits in an Indian clinic.

It is written for endocrinologists, physicians, geriatricians, physiotherapists and dietitians. For background on muscle loss in general, see sarcopenia in India.

Why a Consensus Was Needed

Before 2022, sarcopenic obesity meant different things in different papers. A systematic review run for ESPEN and EASO found 75 eligible studies and marked heterogeneity in how obesity and sarcopenia were defined, how body composition and function were measured, and which cut-offs were used. Prevalence figures could not be compared, and neither could outcome studies.

The response was the ESPEN and EASO consensus statement (Donini et al., published jointly in Clinical Nutrition and Obesity Facts, 2022). It defines sarcopenic obesity as the co-existence of excess adiposity and low muscle mass or function, and lays out a screening, diagnosis and staging algorithm.

Step 1: Screening

Screening needs two things at the same time:

A high BMI or waist circumference, using ethnicity-appropriate cut-offs. The full consensus text points to WHO BMI cut-points and, for waist circumference in Asian populations, to Misra et al. For Asian Indians that means waist of 90 cm or more in men and 80 cm or more in women. Our post on healthy waist size in India covers measurement.

A suspicion of low muscle mass or function: risk factors, clinical symptoms, or a validated questionnaire such as SARC-F in older adults.

The panel also states that everyone over 70 with overweight or obesity should be considered at risk.

Step 2: Diagnosis

Diagnosis has two parts, and the order matters.

2a. Skeletal muscle function first

The consensus names handgrip strength, knee extensor strength, or a chair-stand test (five-times sit-to-stand or 30-second chair stand). It says muscle strength should be the functional parameter of choice. For handgrip cut-offs it points to Dodds et al. for Caucasian populations and to Chen et al., the AWGS authors, for Asian populations. In India that usually means the AWGS 2019 values of under 28 kg in men and under 18 kg in women. Our grip strength normal values guide compares these with Indian-derived cut-offs and covers protocol.

2b. Body composition second

If function is altered, body composition must show both excess fat and low muscle. The consensus prefers DXA, with bioimpedance (BIA) as second choice. The key points:

Fat: raised fat mass percentage, using sex-specific cut-offs. As an example, the panel cites DXA cut-offs of 40% in women and 30% in men that predicted mortality in NHANES data.

Muscle, adjusted for weight: appendicular lean mass divided by weight (ALM/W) is the preferred measure with DXA. With BIA, ALM/W or skeletal muscle mass divided by weight (SMM/W) are acceptable.

Population-specific cut-offs: the panel notes that age, ethnicity and disease may need their own equations and cut-offs.

The weight adjustment is the important difference from ordinary sarcopenia criteria. A larger body carries more muscle in absolute terms, so a height-adjusted index can look normal in someone whose muscle is low for their size. A 2026 study of 205 patients awaiting knee replacement found low muscle mass in 32.7% by an ASM/BMI index against 8.8% by the height-adjusted index. The patients flagged only by ASM/BMI had weaker grip and slower walking before surgery. That was a Japanese orthopaedic cohort, but the logic applies wherever obesity is common.

A result sheet reports muscle and fat side by side, the two halves of the body composition step

Step 3: Staging

Stage

Definition (ESPEN–EASO 2022)

Stage I

No complications attributable to altered body composition and skeletal muscle function

Stage II

At least one complication attributable to altered body composition and skeletal muscle function

Staging gives you a reason to act early in Stage I, before complications appear, and a reason to treat Stage II as an established clinical problem. A 2026 review of 72 studies that used the algorithm found staging was reported in only 19% of them, most often as Stage II. It also found BIA was the most common body composition method and handgrip the most common function test.

What the Asia-Oceania Consensus Adds

In 2025 the Asia-Oceania Association for the Study of Obesity and the International Association of Gerontology and Geriatrics Asia/Oceania Region published their own definition and diagnostic criteria. It uses a three-step algorithm of screening, diagnosis and intervention. Screening uses BMI and waist circumference for obesity, and calf circumference, SARC-F or the finger-ring test for sarcopenia. Diagnosis covers muscle mass by DXA or BIA, handgrip strength, and physical performance (gait speed, SPPB or five-times sit-to-stand). Notably, it treats central obesity as the main concern when diagnosing the obesity half, which suits Asian patients, who tend to carry more visceral fat and develop cardiometabolic risk at lower BMI, as a 2026 Nature Reviews Endocrinology perspective sets out.

ESPEN–EASO 2022

Asia-Oceania 2025

Screen for obesity

BMI or waist circumference

BMI and waist circumference; central obesity emphasised

Screen for low muscle

Risk factors, symptoms, SARC-F

Calf circumference, SARC-F, finger-ring test

Function

Strength preferred (grip, chair stand)

Grip strength plus physical performance

Body composition

DXA, then BIA; high FM% with low ALM/W or SMM/W

DXA or BIA

Third step

Staging (I and II)

Intervention

What Indian Data Show

India has no national prevalence figure for sarcopenic obesity, and most published studies use different definitions. Treat each number as a signal from one setting:

A 2024 Gujarat study of 750 adults from medical camps found sarcopenic obesity in about 40% of those with type 2 diabetes against 11% of non-diabetic adults. It defined sarcopenic obesity as AWGS sarcopenia plus BMI of 25 or more, not by the ESPEN–EASO algorithm.

A 2026 Aligarh hospital study of 151 obese adults over 40 with type 2 diabetes reported 44% prevalence, linked to low activity, poor glycaemic control, insulin use and diabetes complications.

A 2026 BMJ Open meta-analysis of Indian adults with type 2 diabetes pooled sarcopenic obesity at 25.2% across four studies, and rated the certainty of all its prevalence estimates as very low.

For comparison, a 2026 global meta-analysis of studies using the ESPEN–EASO definition pooled prevalence at 11%, with very high heterogeneity. The honest summary: the condition looks common in Indian diabetes clinics, but the numbers depend heavily on the definition and the sample. For the diabetes-clinic workflow, see our diabetes clinic guide.

A Practical Clinic Workflow With BIA

Flag patients with raised BMI or waist plus a risk factor: age over 70, diabetes, recent hospitalisation, reduced mobility, or weight loss on medication.

Run SARC-F or ask about falls, stairs and rising from a chair.

Test grip with a fixed protocol, or do a five-times chair stand.

If function is low, scan. Record percent body fat and a weight-adjusted muscle index (SMM/W or ALM/W). Use the same device each time.

Stage by checking for complications you have already documented.

Refer for dietary review and supervised resistance training, and re-test on a fixed schedule.

The skeletal muscle mass guide explains the muscle outputs on an InBody sheet,. Patients with normal BMI but high body fat follow a different pattern, often called thin-fat.

Limits You Should Know

BIA estimates muscle from equations. The consensus itself warns that BIA-derived muscle is based on equations developed against DXA, which may limit reliability, and that equations need validation.

Hydration in obesity. The panel notes that obesity affects the constant-hydration assumption behind BIA and can reduce its accuracy. An ECW/TBW check helps; see our ECW ratio guide.

Cut-offs are not settled for Indians. Neither consensus publishes validated Indian SMM/W or fat-percentage thresholds for BIA. State which cut-offs you use.

A scan does not diagnose on its own. Function testing and clinical judgement come first.

Frequently Asked Questions

What are the ESPEN–EASO criteria for sarcopenic obesity?

Screen with a high BMI or waist circumference plus clinical suspicion or a positive questionnaire. Confirm with altered muscle function (preferably strength), then body composition showing high fat percentage with low muscle mass adjusted for weight. Stage as I (no complications) or II (at least one complication).

Can bioimpedance be used to diagnose sarcopenic obesity?

Yes, as the second-choice method after DXA. The consensus accepts ALM/W or SMM/W from BIA, while warning that BIA relies on equations and can be less accurate in obesity because of hydration differences. Use the same device for follow-up.

Why adjust muscle for body weight instead of height?

People with obesity carry more absolute muscle, so a height-adjusted index can look normal even when muscle is low relative to the body it has to move. Weight-adjusted indices catch that mismatch.

Which waist cut-offs apply to Indian patients?

The consensus refers to Misra et al. for Asian populations. For Asian Indians that is 90 cm or more for men and 80 cm or more for women.

How common is sarcopenic obesity in India?

There is no national figure. Single-centre and camp studies in people with type 2 diabetes report roughly 25% to 44%, using varied definitions, and a 2026 meta-analysis rated the certainty as very low.

Is sarcopenic obesity the same as thin-fat or normal-weight obesity?

No. Sarcopenic obesity requires excess adiposity and low muscle function and mass, usually with a raised BMI or waist. Thin-fat describes high body fat at a normal BMI and may or may not include low muscle.

{
  "@context": "https://schema.org",
  "@type": "FAQPage",
  "mainEntity": [
    {
      "@type": "Question",
"name": "What are the ESPEN–EASO criteria for sarcopenic obesity?",
      "acceptedAnswer": {
        "@type": "Answer",
"text": "Screen with a high BMI or waist circumference plus clinical suspicion or a positive questionnaire.
